Wellness Checks

An Annual Adult Wellness Check is a yearly 

appointment intended to detect, address, and

prevent chronic and/or life threatening diseases.

During a wellness check a personalized treatment plan is created or updated.

 

This plan may include risk factors to be aware of. Wellness checks differ from annual physicals as annual physicals are more extensive and time consuming. Wellness checks help to your current state of health and create a baseline to compare to and to create a  plan for future care.

What to expect during a wellness check

  • Measure height, weight, blood pressure, and other routine measurements

  • Review medical and family history

  • Create/Update a health risk assessment

  • Address risk factors and treatment options

  • Update list of providers and prescriptions

  • Look for signs of cognitive impairment including diseases such as Alzheimer's and any form of dementia

  • Schedule additional screenings for appropriate preventative services.

  • Screen for depression, anxiety, or any other psychological disorders

  • Provide health advice and referrals to health education and preventative counseling such as weight loss, physical activity, smoking cessation, fall prevention nutrition, cognitive deterioration, etc

Vision screens

Adult Medicine

 
 

Vision screening, also known as an eye test,  is an efficient and cost-effective method to determine your visual acuity and identify potential visual problems and eye disorders. 

 

Vision screens using a Snellen Chart cannot diagnose vision problems, but any abnormalities that may be detected during a vision screen, then a referral will be made to your eye care specialist for diagnosis and treatment. 

Adult Physicals

 

Annual physical exams are extensive yearly appointments meant to detective the possibility of diseases or disorders. During each exam a personalized treatment plan is created to determine the best method of treatment. 

Personalized treatment plans are created per each patient, taking multiple facets of the patient including age, gender, race, culture, and familial history. 

What to expect during an Adult Physical

  • Measure height, weight, blood pressure, and other routine measurements

  • Measure Basic Metabolic Intake (BMI)

  • Review your medical background and family history

  • Assess risk factors for preventable diseases

  • Check vital signs

  • Perform head and neck exams

  • Perform abdominal exams

  • Perform neurological exam

  • Test reflexes

  • Urine testing & blood testing if needed
