Have an issue with a prescription?

We understand how hard it can be to get a pharmacy to correctly provide your Prescriptions. We often have the same issues ourselves. Please indicate the issues you are having and fill out the form below. We will do our best to ensure your medications are sent to your pharmacy, but we can't fill them.


If you require a refill or new prescription we may need to schedule a follow-up appointment with a provider. This is a requirement for most insurance providers and is outside our control.
